Lens flare!!!! You know what's funny, if you watch the clip about 25 seconds in where they are opening Khan's sleeper chamber, watch McCoy. He reaches down and is fussing with something. Go back and watch the whole scene and you'll see Kirk's phaser fall off his waist when he breaks the glass. Kelly is reaching down to pick it up and then decides to just sort of leave it. A classic left in blooper. :)
